Starting with the most recent first, please provide a full employment history. Employers name, address and telephone number, dates of employment, position held, reason for leaving and details of salary or hourly rate are required. Please list and explain any breaks between jobs.
References will be obtained following your verbal approval. Please submit full names and work addresses of three referees who have consented to be approached for a reference on your behalf. At least one referee should be your present employer or if you are not currently employed your most recent employer. Please ensure that your referees are informed if you have changed your name since they last employed you.
Please provide the title, full name, address, phone number, email address and your relationship for each referee.
All appointments are subject to a satisfactory health screening. If there is anything in your medical history that may affect your ability to perform your duties, you will be required to provide further details.
Any information submitted will be treated as strictly confidential.
Because of the nature of the duties of the post that you are applying, the position is exempt from The Rehabilitation of Offenders Act 1974, (Exceptions) Order 1975. All applicants offered employment will be subject to an Enhanced Check which contains details of an individual's convictions, cautions, reprimands or warnings recorded on police central records and includes both 'spent' and 'unspent' convictions together with any information held locally by police forces that it is reasonably considered might be relevant to the post applied. An Enhanced Check with information from the DBS's children and adults barred list is for those individuals engaged in regulated activity with both vulnerable groups including children and a small number of posts as listed in the Police Act regulations. Only relevant convictions and information will be taken into account so disclosure may not necessarily be a bar to obtaining this position All information submitted would be treated in the strictest confidence. This means that all applicants are obliged to provide full details of any convictions (however old), cautions, reprimands and final warnings including those that are now considered "spent".
